The Aspace Catalonia Foundation has participated in the 26th International Conference on Integrated Care (ICIC26), one of the main global reference spaces in integrated care, held in Birmingham and with more than 1.500 professionals, researchers and public policy makers from around the world.
Beyond institutional presence, Aspace Catalunya has shared a specific way of understanding care, connected, person-centered and aimed at generating real changes in the lives of people with complex neurodevelopmental disorders, cerebral palsy and multiple disabilities.
Sharing a model that responds to global challenges
At a time when health and social systems are facing the challenge of overcoming fragmentation and responding to highly complex situations, Aspace Catalunya has provided a model of more than 65 years of experience built on direct care and work with people and families.
Tamara Biedermann Villagra, director of the Health Sector, and Carlos Varela Ferro, director of Transversal Operations and the Social Sphere, led the participation with two oral communications that not only explained what Aspace Catalunya does, but also how attention can be transformed from within organizations.
Technology at the service of more coordinated and anticipatory care
One of the presentations, "Aspace Digital Care: A Data-Driven Platform for Integrated and Person-Centred Support in Complex Disability Care", has presented a digital solution based on a connected ecosystem that integrates health, social and educational information in real time.
This system is not just a technological tool: allows professional teams to share information in an agile manner, anticipate risk situations and reduce avoidable interventionsIn practice, this translates into fewer repeated tests, less lack of coordination between services and a smoother relationship with families, who take a more active role.
Putting the person at the center to break fragmentation
The second communication, "Building a Person-Centred Integrated Care Model for People with Complex Disabilities", has delved into the organizational model that makes this transformation possible.
Aspace is committed to interdisciplinary teams working in a coordinated manner, with shared leadership and with the real participation of families in decision-making. This implies moving from a fragmented model, where each service acts independently, to a model where the person and their life project are the connection point of all supports.
This approach allows respond to a common reality, people who need simultaneous attention in health, social and educational areas, but who often encounter systems that do not talk to each other.
Tangible impact on people's lives
The data presented at the congress show specific results, improvement of continuity of care, increased satisfaction of families and professionals and reduction of avoidable situations through more coordinated care, with a preventive and predictive approach.
Beyond the indicators, This model contributes to improving people's quality of life, less uncertainty, more coherence in interventions and care that accompanies throughout life, adapting to each moment.
With more than 64 years of experience and serving more than 3.700 people annually, Aspace Catalunya builds this model from daily practice, integrating knowledge, experience and commitment.
Looking to the future, leading from practice
La Participation in ICIC26 reinforces the Foundation's international projection, but above all it consolidates an idea, the transformation of systems does not just involve designing new models, but rather demonstrating that they work in reality.
Aspace Catalunya continues to advance in this line, contributing to the global debate with solutions that combine technology, organization and a person-centered perspective.
Because transform attention it's not just about connecting data or services, but ensure that each person receives more coherent, more anticipated and more humane care.
