The annual meeting of the Catalan Union of Hospitals was held today, which has already established itself as a reference meeting for the health and social sector. This year it aims to take a further step, making visible the maturity and impact of the +FUTUR Project with the implementation of healthcare integration. The entities associated with La Unió are acting to move forward, with experiences of transformation in our organizations.
The Conference took place with a more innovative format than ever before. With the emotion of the testimonies of specific cases from different entities and territories, participation and opinion of the public, as well as with a collection of recommendations to take home on how to manage change, towards a real care model that promotes attention focused on the person thanks to the implementation of care integration.
Care integration means coordinated and oriented services so that people receive a continuum of care based on their needs and throughout the life course.
To make the implementation possible, four key points must be kept in mind:
- Agreements and territorial governance for healthcare integration
- Design of transversal processes and circuits
- Communication tools and skills for professionals
- Participation of individuals and families
In this sense, the Fundació Aspace Catalunya presented at the +Future Conference of the UCH, our experience in making possible the active participation of people in care and families.
