The patient's affiliation and contact details are very important to facilitate contact between the patient and the Foundation. For this reason, we must be rigorous in modifying them.
Fill in the following form to update the contact details (change of address, telephone, etc.) contained in a patient's medical history.
We recommend that you also communicate this change to your primary care center (CatSalut). In the event that the data contained in the Foundation's clinical history do not match those contained in the CatSalut register, those contained in CatSalut may be taken as a reference.
